FL Region Event Affiliate Application - 25/26
Please use this form to renew your USAV affiliation for the 2025/2026 season. All affiliates are required to renew each USAV season. Please have all items required at the time of renewing. Contact the Events Department at events@floridavolleyball.org if you have any questions regarding renewal.
